What Are Individual Support Services?
Individual Support Services (ISS) are state-funded in-home supports designed to help adults with intellectual and developmental disabilities develop the skills needed to live as independently as possible. ISS focuses heavily on teaching and reinforcement rather than passive care.
Day-to-day, ISS looks like structured, one-on-one coaching and assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Our DSPs assist with:

Who Qualifies for ISS in New Jersey
DDD Enrollment & Age
The individual must be a New Jersey resident, meet Medicaid eligibility criteria, have a documented developmental disability, and be 21 years of age or older.
NJCAT Assessment
The individual must complete the New Jersey Comprehensive Assessment Tool (NJCAT) to determine their specific support needs and generate their annual budget.
PCSP Authorization
